Increases the rate at which the esophagus, stomach, and intestines move during digestion.
It also increases the rate at which your stomach empties into your intestines and increases the strength of your lower esophageal sphincter.
Unamol is used to treat gastric reflux (the regurgitation of stomach acid into the esophagus), which is generally experienced as heartburn. Unamol can also be used for purposes not listed in the Unamol guide.
Unamol Pharmacology
Unamol tablets and suspension contain cisapride as the monohydrate, which is an oral gastrointestinal agent chemically designated as (±) -cis-4-amino-5-chloro-N- [1- [3- (4-fluorophenoxy) propyl] -3 -methoxy-4-piperidinyl] -2-methoxybenzamide monohydrate.
Its empirical formula is C23H29ClFN3O4 · H2O. The molecular weight is 483.97.
Cisapride as the monohydrate is a white to slightly beige odorless powder. It is practically insoluble in water, sparingly soluble in methanol, and soluble in acetone. Each 1.04 mg of cisapride as the monohydrate is equivalent to one mg of cisapride.
Unamol is available for oral use in tablets containing cisapride as the monohydrate equivalent to 10 mg or 20 mg of cisapride and as a suspension containing 1 mg/ml of cisapride.
The inactive ingredients in the tablets are colloidal silicon dioxide, lactose monohydrate, magnesium stearate, microcrystalline cellulose, polysorbate 20, povidone, and starch (corn).
The 20 mg tablets also contain FD&C blue no. 2 aluminum lake.
The inactive ingredients in the suspension are:
- Hydroxypropyl methylcellulose, methylparaben, microcrystalline cellulose, sodium carboxymethyl cellulose, polysorbate 20, propylparaben, sodium chloride, sorbitol, and water.
- The 1 mg/ml suspension contains artificial cherry cream flavor and FD&C red no. 40.
Unamol uses
For the symptomatic treatment of adult patients with nocturnal heartburn due to gastroesophageal reflux disease.
Unamol is a parasympathomimetic that acts as a serotonin 5-HT4 agonist. Stimulation of serotonin receptors increases the release of acetylcholine in the enteric nervous system.
Unamol stimulates the motility of the upper gastrointestinal tract without stimulating gastric, biliary, or pancreatic secretions.
Unamol increases the tone and amplitude of gastric contractions (especially antral), relaxes the pyloric sphincter and the duodenal bulb, and increases the peristalsis of the duodenum and jejunum, which produces accelerated gastric emptying and intestinal transit.
Lower esophageal sphincter resting tone increases. It has little or no effect on the motility of the colon or gallbladder. Unamol does not induce muscarinic or nicotinic receptor stimulation, nor does it inhibit acetylcholinesterase activity.

Unamol Studies
The effect of Unamol (20 mg twice daily), a new prokinetic drug, on bowel habits and laxative consumption in patients with idiopathic painless constipation and chronic laxative intake was studied.
After a baseline period of four weeks, spontaneous defection (frequency without laxative ingestion) and total defecation (total frequency) were measured.
Patients with spontaneous excretion of fewer than three stools per week entered the treatment period and were randomly assigned to double-blind treatment with Unamol (n = 64) or placebo (n = 62).
After eight weeks of treatment, a four-week single-blind placebo medication administration phase was performed.
Unamol and placebo increased the frequency of spontaneous stool from 1.1 +/- 0.2 SEM to 3.0 +/- 0.2 per week (p less than 0.001) and from 1.2 +/- 0.1 to 1.5 +/- 0.2 (p greater than 0.05 ), respectively.
Laxative consumption decreased from 3.6 +/- 0.3 to 1.8 +/- 0.2 doses / week for Unamol (p less than 0.001) and from 3.3 +/- 0.3 to 2.8 +/- 0.3 for placebo (p less than 0.05).
Both drugs improved constipation as assessed by the patient using a visual analog scale, but Unamol did so more significant than the placebo.
The effects of cisapride partially survived the active medication for at least four weeks.
It is concluded that cisapride improves bowel habits in patients with idiopathic constipation and reduces the consumption of laxatives.
Unamol dosage
Take exactly as prescribed by your doctor. Do not eat larger or smaller amounts or for longer than recommended. Follow the directions on the medicine label.
Usual adult dose for gastroesophageal reflux disease:
10 mg orally four times a day, 15 minutes before meals and bedtime. It can be increased to 20 mg per dose if necessary.
Usual adult dose for gastroparesis:
10 mg orally 4 times a day, 15 minutes before meals and bedtime. It can be increased to 20 mg per dose if necessary.
Usual adult dose for dyspepsia:
5 mg orally three times a day 15 minutes before meals. It can be increased to 10 mg per dose if necessary.
Usual pediatric dose for gastroesophageal reflux disease:
Older than 1 year: 0.2 to 0.3 mg / kg / dose orally 3 to 4 times a day. Maximum 10 mg per dose.
Shake the oral suspension (liquid) well before measuring a dose. Measure the liquid with a special dose-measuring spoon or medicine glass, not a regular spoon. Ask your pharmacist for one if you don’t have a dose-measuring device.
Unamol is generally taken four times a day, at least 15 minutes before meals and bedtime. Follow the instructions of your doctor. Store at room temperature away from moisture and heat.
Can Unamol use be stopped immediately or gradually?

Unamol side effects
In many countries (including Canada), Unamol has been withdrawn, or its indications have been limited due to reports of long QT syndrome due to Unamol, which predisposes to arrhythmias.
The Food and Drug Administration issued a warning letter regarding this risk to healthcare professionals and patients.
Seek emergency medical attention if you have any of these signs of an allergic reaction: hives, trouble breathing, swelling of your face, lips, tongue, or throat.
Serious cardiac arrhythmias, including ventricular tachycardia, ventricular fibrillation, torsades de pointes, and QT prolongation, have been reported in cisapride patients (Unamol).
Stop using Unamol and call your doctor at once if you have severe side effects, such as:
Fast or irregular heartbeat; or feeling like you might pass out. History of irregular heartbeat, abnormal EKG, heart disease, lung disease, dehydration, or persistent vomiting.
Less serious side effects may include:
Diarrhea, nausea, headache, nasal congestion, constipation, increased urination, or cough may infrequently occur as your body adjusts to the medication. Tell your doctor if any of these effects persist or get worse immediately.
Notify your doctor if you have vision changes, chest pain, mental/mood changes, or stomach pain while taking this medicine. This is not a complete list of side effects, and others may occur.
Who should not take Unamol?
You should not use Unamol if you are allergic to it or if you have:
- Bleeding or blockage in your stomach; heart disease or hardening of the arteries; congestive heart failure; slow heart rate or a heart rhythm disorder.
- A personal or family history of “long QT syndrome”; a structural heart defect; heart block or other conduction disorders; severe dehydration.
- Malnutrition, an eating disorder; renal insufficiency; severe lung problems, or advanced cancer.
Do not take Unamol without first consulting your doctor if you are taking any of the following medications:
- Antibiotics such as clarithromycin, erythromycin (Ery-Tab, EES, E-Mycin, others), antidepressants such as amitriptyline (Elavil, Vanatrip), or nefazodone (Serzone).
- Antifungal medications such as fluconazole (Diflucan), itraconazole (Sporanox), and ketoconazole (Extina, Ketozol, Nizoral, Xolegal).
- Phenothiazines such as prochlorperazine (Compazine, others) and promethazine (Phenergan, others).
- Heart medications such as procainamide (Procan SR, Procanbid, Pronestyl) and quinidine (Quin-G); or HIV medications such as indinavir (Crixivan) and ritonavir (Norvir, Kaletra).
To make sure you can safely take Unamol, tell your doctor if you have kidney or liver disease.
Can Unamol be taken or consumed during pregnancy?
The Food and Drug Administration rates Unamol pregnancy category C. It is not known whether Unamol will harm an unborn baby. Please visit your doctor for a recommendation as this case requires special attention.
Can Unamol be taken for nursing mothers or while breastfeeding?
Unamol can pass into breast milk and may harm a nursing baby. Do not use this medicine without telling your doctor if you are breastfeeding a baby. Kindly explain your condition and condition to your doctor and seek expert medical advice.
What other drugs will affect Unamol?
The sedative effects of benzodiazepines and alcohol can be enhanced. Prothrombin time in patients on anticoagulants may increase. Effect antagonized by anticholinergic drugs.
Avoid Unamol with antiallergics, antibacterials, antidepressants, antifungals, antinauseants, antipsychotics, and protease inhibitors. Unamol increases the gastrointestinal absorption of cimetidine and ranitidine.
Grapefruit juice increases the bioavailability of cisapride; avoid concomitant use. Before using Unamol, tell your doctor if you regularly use other medications that make you drowsy, such as:
- Cold or allergy medications, sedatives, narcotic pain relievers, sleeping pills, muscle relaxers, and medicines for seizures, depression, or anxiety. They can add drowsiness caused by Unamol.
Tell your doctor about all other medicines you use, especially:
- Belladona (Donnatal); cimetidina (Tagamet, Tagamet HB) diciclomina (Bentyl); clidinio (Quarzan); hiosciamina (Levsin, Cystospaz, Anaspaz); propantelina (Pro-Banthine); o un anticoagulante como warfarina (Coumadin).
